Doctors can give better quality care when they know as much as possible about their patients including medical history as well as values and beliefs about health and illness.
Whenever I visit a new doctor, I have to start over, from the beginning, and go by memory. What always happens is I leave stuff out, or forget things, also hate writing this over and over.
This simple Word doc takes just a few minutes to fill out. And, as you get older, you can keep it up to date. Each time you go to a doctor’s office just print it out and bring it with you. Keep one where you can find it. Make sure your kids or parents have a copy as well.
Over time, doctors will tell you what you should add or leave out for the next doctor, as you take charge of doing a better job of telling your doctor what your doctor needs to know.
